  •  Claim Form duly completed.
  • Report of the attending doctor.
  • Investigation report such as laboratory test, X-ray & confirmation report of injury
  • Medical bill corresponding to doctor prescription where medical extension is granted
  • Police Report/Muchulka.
  • Record of Admit and Discharge from the hospital.
  • Salary Statement
  •  Attendance Record.
  • Post-Mortem Report. (In case of death due to accident)
