- Claim Form duly completed.
- Report of the attending doctor.
- Investigation report such as laboratory test, X-ray & confirmation report of injury
- Medical bill corresponding to doctor prescription where medical extension is granted
- Police Report/Muchulka.
- Record of Admit and Discharge from the hospital.
- Salary Statement
- Attendance Record.
- Post-Mortem Report. (In case of death due to accident)
